Ability to add notes onto Workflow task

Under each element of the workflow, have the ability to add notes, so that if a step is marked as NA, a note can be added to confirm why this task was not required. Or enable note to be added, to prove that steps have been completed (e.g. writing the bank balance, when a bank has been reconciled etc)
