Include form name in form approval e-mail template

At the moment the e-mail template just says a document is waiting for approval. The e-mail address may be used by the client for several businesses, so the e-mail template should state the business name and the template/document name.

On Task screen- Show linked Documents

When uploading a Document you can link a Task to it. But this does not appear on the task. Would be good to include a link to these Documents from the task screen.
